In this role, you will be responsible for delivering specialized pharmaceutical care tailored to paediatric patients. This role involves managing prescription medication, providing drug information, participating in training and development, engaging in quality improvement projects, and ensuring operational and administrative efficiency. The Paediatric Pharmacist will work collaboratively with healthcare providers to ensure optimal medication therapy and patient safety.

 

Job Description:

Prescription Management

  • Accurately fill and dispense paediatric prescriptions in accordance with legal, regulatory, and hospital standards.
  • Review and verify prescriptions and doctors' orders to ensure appropriate therapy, dosage, and administration routes for paediatric patients.
  • Conduct comprehensive medication reviews to identify potential drug interactions, contraindications, and necessary dosing adjustments specific to paediatric patients.
  • Collaborate with healthcare providers to optimize medication therapy and enhance patient safety.
  • Educate patients and their families about medications, including proper usage and potential side effects.

Inpatient Drug Review

  • Review and manage medication orders for paediatric patients, ensuring safety and efficacy.
  • Collaborate with healthcare providers to optimize pharmacotherapy and provide drug information.
  • Monitor patient responses to medications and adjust therapy as needed.

Drug Information

  • Provide accurate, current drug information regarding paediatric medications to physicians, nurses, patients, and caregivers.
  • Educate patients and caregivers on medication use, including potential side effects and proper administration techniques.

Training & Development

  • Assist in training and developing pharmacy technicians to ensure they are proficient in managing paediatric medications.
  • Provide ongoing education and support to pharmacy staff on best practices and advances in paediatric pharmacy.
  • Engage in clinical research and contribute to the education of pharmacy students and residents.

Work Improvement Projects

  • Participate in quality improvement initiatives aimed at enhancing pharmacy operations and patient care.
  • Contribute to the development and implementation of new protocols and best practices in paediatric pharmacy services.

Operational & Administrative Duties

  • Manage inventory, including the ordering, receiving, and stocking of paediatric medications.
  • Ensure adherence to hospital policies, regulatory requirements, and best practices in all pharmacy operations.
  • Review of SOPs/policies/procedures e.g. Paediatric BMT Nutrition SOP with paediatric dietician.
  • Validated EPIC medication order builds for paediatrics for NCID Outbreak.
  • Assist in maintaining accurate patient records, including prescription and billing documentation.
  • Participate in the development and maintenance of paediatric formularies.

Patient Safety & Quality Assurance

  • Implement and monitor safety protocols to minimize medication errors and ensure the highest standards of patient care.
  • Participate in multidisciplinary meetings to address and resolve medication-related concerns.

 

Job Requirements:

  • Doctor of Pharmacy (Pharm.D.) / Degree in Pharmacy or equivalent, from an accredited institution; recognised by the Singapore Pharmacy Board for registration.
  • Registered with Singapore Pharmacy Council.
  • Clinical pharmacy experience in a paediatric setting preferred.
  • Experience with medication therapy management and patient counselling is highly desirable.
  • Preferably 1 years’ experience in hospital.
  • Strong analytical and problem-solving abilities.
  • Excellent verbal and written communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Proficiency in using electronic health records (EHR) and pharmacy management systems.
  • Knowledge of paediatric pharmacotherapy and drug interactions.
  • Computer literate with knowledge of Microsoft Word, Excel and PowerPoint.
  • Must be fully vaccinated with COVID-19 vaccinations.
