Training Officer
IDHealth, an initiative of Happee Hearts Movement is a community-based, integrated health team dedicated to serving adults with intellectual disability and their caregivers.
People with intellectual disability have poorer health and lower access to and use of healthcare services than the general population. IDHealth’s goal is to reduce the barriers to health and healthcare utilization for this population by providing a health service that is accessible to and appropriate for people with intellectual disability. In addition, we aim to increase the capability of Singapore’s health and social service sectors to serve this population.

Roles and Responsibilities:
Clinical
- Participate actively in interdisciplinary team discussions on patient care
- Support clinical team in implementing and monitoring treatment interventions (e.g., medical, behavioural, communication, engagement, transitions etc.) based on the care plan derived. This will require the individual to work closely and in collaboration with IDHealth team members, volunteers as well as external partners and service providers across the health, disability, and social sectors. In addition, the individual must be prepared to work in a variety of physical locations (e.g., clinic, home visits, DACs), and effectively use different modes of communication (e.g., phone call, video call, email etc.).
- Support clinical team in escorting patients for their medical appointments when necessary.
- Maintain clear documentation of interaction with PWIDs, caregivers, families, service providers and volunteers.
Advocacy and Capability Building
- Participate actively in team advocacy events.
- Be prepared to explain and advocate for the work of IDHealth to health, disability and social service providers.
Others
- Carry out other duties as assigned from time to time.
Requirements:
- Minimum ‘O’ Level/’A’ Level/ITE Certification and above, from a recognised educational institution
- Relevant experience in the fields of Special Education or Social services is preferred
- Basic proficiency in Microsoft Word and Powerpoint
- Ability to work well within a team and with other stakeholders
- Ability to follow through on assigned tasks
- Effective verbal and written communication skills
- Effective problem-solving skills
- Physically fit
