Wait-List Transplant Coordinator
The wait-list coordinator ensures coordination of the clinical aspects of transplant patient care through the waitlist for deceased donor kidney transplantation and transition phase of deceased donor kidney transplantation. This include (1) receiving referrals and making first contact with the patient, (2) perform interim assessment of patient’s suitability to be placed on the wait-list, (3) schedule tests and specialist visits that are required for placement on the wait-list, (4) ensure patient is reviewed at the wait-list registration or wait-list maintenance clinic, (5) provide counselling of patient with respect to being on the wait-list, (6) monitor patient condition and testing during the wait-list phase and (7) maintain database with intent to report wait-list management data on a regular basis. The wait-list coordinator will also handle any calls or concerns from wait-listed patients, monitor the condition of wait-listed patients that are admitted and ensure that the waitlist is upkeep accurately at all times so that patients from SGH will always have the best opportunity to receive a deceased donor offer. The wait-list transplant coordinator will also be expected to take turn to do overnight calls for deceased donor transplant activation where the coordinator is in charge of preparing patients for emergency deceased donor kidney transplantation. The wait-list coordinator will work collegially with other healthcare professionals to ensure holistic care of the waitlist and perform any other duties requested by the transplant program manager or director which include roles in quality improvement, education and research. A candidate suitable for this job post would be someone who has interest in working in the healthcare industry, is a team player, able to be patient-centric and most importantly enjoy interacting and helping others.
Requirement
1.Degree holder in Health Care Related or Life Sciences or Psychology
2.Candidates will prior experience in healthcare sector will be an added advantage
