Care Coordinator (Orthopaedics)
You will assist the HOD, Programme Director and Programme Manager in providing operational, administrative, financial and project management support to mainly the Orthopaedic Integrated Clinical Pathways (OICP) Programme in JurongHealth Campus.
Responsibilities
- Assist and support Programme Director, Programme Manager and the respective Clinician Leads in the implementation, development and management of the programme.
- Coordinate care across the various care settings and act as patient advocate between patients, caregivers and hospital (including telephonic follow-up post operation).
- Organise regular multidisciplinary team meeting (including case discussion) and ensure appropriate documentation at all time.
- Develop, enhance and manage new initiatives or clinical services that enhance the clinical outcomes and patient experience of the programme.
- Manage projects as assigned independently through engagement efforts and collaborations with internal and external stakeholders.
- Plan, coordinate and facilitate in the engagement sessions, publicity activities and collaboration efforts between hospital, internal and external partners.
- Support data management, report analysis and performance management of the programme and projects as assigned.
- Provide administrative and secretariat support as assigned.
- Undertake any other duties as assigned.
Job Requirements
- Bachelor’s Degree from a recognised university.
- Minimum 1-year of work experience, preferably from the healthcare related industry. Working experience in Case Management will have an added advantage.
- Proficient in Microsoft Office Applications.
- Knowledge of statistics.
- Good communication skills both written and spoken.
- Ability to work independently and work as a team.
- Pro-active and self-motivated.
- Good interpersonal relations skills.
