- Work closely with internal stakeholders to support smooth delivery of homecare services.
- Assist in managing client inquiries and coordinating requested services.
- Participate in case discussions and multidisciplinary care planning.
- Assist in coordinating referrals with internal teams and external community partners.
- Participate in home visits with care teams to observe and support client assessment, care delivery, and follow-up.
- Engage with clients and caregivers during home visits and telephone follow-ups to understand needs and challenges.
- Support caregiver education by reinforcing care techniques, resources, and available support services.
- Collaborate with social and healthcare partners to recommend appropriate care programmes.
- Assist in discharge planning coordination with Operations and Nurse Managers.
- Support the analysis of backend statistical data to identify insights and enhance client and caregiver experience.
- Actively support and participate in shaping development of programme development, evaluation, and service improvement initiatives.
- Contribute to service innovation or pilot projects to enhance care delivery.
Qualifications
You are:
- Currently pursuing an undergraduate degree, diploma, or ITE. (Healthcare, Social Work, Psychology, Public Health, Business, or related fields preferred)
- A team player with strong communication and stakeholder management skills.
- Detail-oriented, proactive, and eager to learn.
- Proficient in Google Suite applications.
Other Information
Weekend and/or after office support may be required to support engagement and program efforts and initiatives

- To be exposed to community-based healthcare and integrated care models in Singapore.
- To be able to understand and support client assessment and care coordination processes.
- To apply communication and interpersonal skills when engaging clients and caregivers.

- To learn how to support caregivers through education, engagement, and resource navigation.
- To gain knowledge in programme development, service evaluation, and community care delivery.
